The Form widget was built based on the Exam, but it's meant for collecting input in a structured way where you don't score answers; instead, it allows you to collect information that can be tied to a specific user without sharing that information to other participants.
One example use case is to use the Form to gather feedback from participants after a course or some event.
Field types include: single choice, multiple choice, dropdown, scale, sorting, text, number and date.
Editing the widget

Fields
There are eight field types: single choice, multiple choice, dropdown, scale, sorting, text, number, and date. Below is an explanation for each type along with examples.
Single choice
With this field type, you can ask a question that has multiple answer options, out of which participants can select only one. To create a single-choice question:
Press "Add field"
Select "Single choice"
Fill in the question you want to ask in the title field
You may also give additional instructions or context in the description field,

Multiple choice
With this field type, you can ask a question that has multiple answer options, giving participants the possibility to choose multiple. To create a multiple-choice question:
Press "Add field"
Select "Multiple choice"

Dropdown
This allows you to create a question with a collapsable dropdown view for answer options - in practice, an option to a single choice question that has different visuals.
To create a dropdown question:
Press "Add field"
Select "Dropdown"

Tick the box "required" to prevent participants from skipping this question, and

Scale
This field type is similar to the Pulse widget, but it allows you to include them in the same Form as other types of questions, allowing for a different type of analysis. Unlike Pulse, you can also rename each option on the scale. To create a scale question:

Sorting
This field type allows you to give participants a number of options, which participants need to drag in some order. To create a sorting-type question:

Select "Sorting"

Text
This field type allows you to ask a question where participants are free to give their own answers in writing. To create one:
Press "Add field"
Select "Text"
Fill in the question you want to ask in the title field

Tick the box "required" to prevent participants from submitting their answers but skipping this question, and
Attach a file for additional context. Common file formats are supported, so you may attach for example a PDF, video, an image, or an audio file.

Number
This field type is similar to text (explained above), but the answer has to be a number. To create one:
Press "Add field"
Select "Number"

Date
This field type allows participants to select a date from a calendar view, e.g. the date they attended the training they're giving feedback for. To create a Date field:
Press "Add field"
Select "Date"

Advanced settings
There are a few additional widget-level settings that you can utilise:
Show fields one by one: instead of showing the entire Form at once, it displays one question and its options at a time, as well as buttons to move back and forth between questions.
Show progress: displays a progress bar at the bottom of the widget, visualising to participants how far they are in the form. This is how it might look:
Allow participants to change answers: gives participants the possibility to change their answers after they've submitted the form.
Form completed description: you can add a message to be shown to participants when they submit their answers.
Form completed attachment: you can add an image or another common file type to be shown to participants when they submit their answers.
Enable permanent anonymity: can be enabled when there are no answers in the widget. Once enabled, the only way to change the setting is to duplicate the widget and change it in the new one. With permanent anonymity, user data is not shown in the widget or in export files.
Who can edit
You can also allow participants to edit the widget by adding their names or email addresses in the "Who can edit" section. Instead of listing individual participants, you can give edit rights to entire userlists.

Results view (admins)
Admins can see a results view showing everyone's answers with the "show all results" option in widget menu. Here's how it might look:
Additionally, you can export Form data to Excel from the "..." widget menu for further analysis. The export file includes profile fields, as well as information on which teams & userlists each respondent belongs to in the workspace.
